What if I never had rehab after my cardiac event?

Give us a call. We will make a recommendation as to the next step you should take. Wellness is the key to a long and independent life-style.​ We have multiple options for patients at the Wellness Center.

How do I participate?

A physician referral will be needed prior to starting. To get started, you should print out our referral form to fill out and take to your physician, and then talk to your physician about participating in cardiac rehabilitation.  We will be glad to fax your physician a referral form to complete for entrance into the program, if you have any trouble printing it.  For additional information, call

Who should participate?

Patients with the following conditions or diagnoses are eligible for cardiac rehab:

  • Heart Attack (within the past 12 months)

  • Coronary Artery Bypass Surgery

  • Angioplasty/Stent

  • Stable Angina

  • Heart Transplant

  • Heart Valve Surgery

  • Stable Chronic Heart Failure (EF ≤ 35%, clinically stable for 6 weeks, & NYHA Class II-IV)

Patients with peripheral artery disease (PAD) may benefit from our Supervised Exercise Therapy (SET) program. Click here for more information on our SET program.

  • Registered Nurses

  • Program Assistants
