Most insurance plans cover monitored sessions.  Our staff will contact your insurance company once you are referred by your physician. We will verify cardiac rehabilitation benefits before you enter the program. Financial assistance is available for cardiac rehab patients. You can complete the forms below and bring the to the Wellness Center.
